Yeah, the 115 is the predecessor to the 1212.

They're actually somewhat common around here as well. Apparently a bunch of them were ordered when the Zimmer Station power plant near Moscow, OH was being built, as it was originally going to be a nuclear plant, but it got converted to coal at some point and all of the sirens were sold off to surrounding municipalities.

Here are the ones I know of:

Amelia, OH - on the BMOP fire station on Main Street
California, KY - on the fire station, 3707 Smith Road
Cold Spring, KY - on the fire station, 4113 Alexandria Pike
Neville, OH - on a pole at the corner of Forest and Coffee
New Richmond, OH - on a pole, Center Street between Walnut and Union
Peach Grove, KY - on a pole, KY-154 access road just east of KY-154
Silver Grove, KY - on the fire station, 5011 4 Mile Road
Southgate, KY - on the city building, 122 Electric Avenue

Notes:
Neville: doesn't work according to Clayton
Southgate: top row of horns turned horizontally; bottom row is vertical
Silver Grove: horns are all turned horizontally instead of vertically

There may be others floating around Clermont County as well.

I never noticed one on the 115 until now, but Federal really loved to slap the Model 2 top onto other sirens, didn't they?

Oh, this one is much larger than a Model 2 housing's top. It looks more like one off of a Model 3 housing, or it could be entirely custom for the 115 and 1212. It also has the crimped edge like the ones on Model 3/5/7 housings.

As of three years ago, there was one atop the Rec IM building at Eastern Michigan University that was still in service.

I have one around here somewhere, though it is in various states of disassembly. The speakers are still in Georgia...should probably go get those.
