There may be a couple in Cincinnati, OH., I know of at least one. There is also one in Witchita, KS. that there have been a couple video of.

There is one in Knoxville, TN, although it is not in use.

I'm sure St. Paul, MN has quite a few, as their system (until April 2012, when it was replaced with 2001-130's), was mostly t-bolt 1000's along with a few STH-10's and Model 5's installed in the early 50's.

Not to say there aren't any for sure, but I have not seen a grid-faced Tbolt in St. Paul. All of the Thunderbolts in St.Paul were single tone 1000s for sure though. A lot of them had black painted rotator boxes.

There is at least one in Cincinnati, and that would be the one on top of Cardinal Pacelli School. Unfortunately, the school would cut the power to it on test days. The T-128 that replaced it is installed on their property, so I wonder how they've been putting up with it.
Resident of a county with big a mixture of sirens, but in the process of being replaced. :(

Here is a map I made for no reason. It deals with the Jailbar Tbolts.
